1) Name & Type of Group (Birthright, Teen Tour, Synagogue Mission)
2) Contact Person & His/Her Contact Information
3) Date of Visit
4) Whether or not the group would like home hospitality for Shabbat meals, and for how many people
5) Whether or not the group would like to arrange for a presentation by Rabbi Pear before or after the group’s visit to shul
