1) Print it out and return it to Shir Hadash (with appropriate payment), either via mail: Shir Hadash / POB 8438 / German Colony, Jerusalem; or by dropping it off at the Shir Hadash office located on the top floor of our Cheyl Nashim #4 location.
OR 2) Fill out the form online and return it via e-mail to shirhadash613@gmail.com; then send in the appropriate payment to the above address or drop it off at the office.
